Failed to notice emergency tax codes which equated to incomes that exceed £100,000pa. Failed to address associated salary sacrifices, perks and fringe benefits within a NFP Charity. Failed to question why my ex was earning less than 10 years previous and why he'd appeared not to have had a pay rise over 4 years. Resulting in what an accountant has confirmed as fraudulent disclosure, non disclosure of relevant facts. Since raising this within a subject Access Request to which I've had no reply Wartnaby Hefford are in the process of updating their online profile.
